Insurance Cover is provided for injuries caused solely by active participation in officially organised games, practice and training sessions and official functions, via AMERICAN HOME ASSURANCE COMPANY.

If you are injured...

1. Obtain a Sports Injury Claim Form from the Victorian Squash Federation office (Ph: 9682 2199) or download a form:
www.squash.org.au/forms/InsuranceClaimForm.pdf
2.Complete the questions on the front page, ensuring that you sign and date the form.
3.Return the claim form to your Club's official for the Club Declaration to be completed by an authorised club official.
4.The claim form must be received by VSF within thirty (30) days of injury, invoices and treatment costs can be sent in at a later date.
5.Claims should be directed initially to your Private Health Insurer (if applicable), then sent to VSF with a copy of your reimbursement from your Private Health Insurer.
6.The insurance policy is to reimburse the player for Non Medicare expenses as per benefit chart. However the responsibility for the initial payment rests with the player.
7.Due to Government Legislation Act, American Home Assurance Company is unable to assist you with Medicare expenses.
8.An attending Physicians Statement form must be completed by your Doctor/Surgeon when claiming, Private Hospital expenses and Loss of Income benefit.
9.Claim forms and expenses are to be sent to:

NB: If an Insured Person suffers an Injury and requires physiotherapy treatment(s), the company will pay Compensation up to the amount of Compensation shown in Section C of the Events Schedule provided that the Insured Person has obtained a referral from a registered and legally qualified medical practitioner. Compensation under Section C of the Events Schedule for physiotherapy treatment(s) will cease as soon as the Insured Person resumes playing sport.

If you are a financial member of your club and are currently registered to play competition pennant with the VSF or any of its affiliate associations you are covered.

IMPORTANT NOTE:

Please be advised the age limit on the player accident scheme has been increased from 70 to 75yrs of age, although negotiations are still being held to discuss removal of any age limit.
